Elmah not logging (MS SQL) and not sending email

I'm running a web application ASP.net on IIS 8.5 (Windows Server 2012).

I've add the Elmah.dll to my bin folder, then changed my web.config with this configuration (this is my complete web.config):

I've also created in my database the ELMAH error table with the creation script provided on Elmah website.

I've checked the authorizations on the stored procedures.

Then I've started my application and encountered an error 500, but no error was logged on the database and also no email is send to my email address.

Any suggestion?
